| Terms and Conditions of Business and Patient - Dentist Agreement for Durban Dental Centre. Our Commitment to Patient Care We aim to treat all of our patients with kindness and respect and ensure every visit is as comfortable and relaxing as possible. We will give a written quotation of the cost in advance of any major treatment. We undertake to do our very best treatment for you. We will listen to you when you talk. We will apologise and make every effort to improve if we have not done as we promised. We will do our absolute best to run to time, as we know how important your time is to you. Our requirements of our Patients We ask that you attend for your appointment in good time, so as to not keep others waiting and to allow for any paper work to be completed prior to your treatment. We ask you to accept that if you arrive too late for us to be able to complete the planned treatment, that we reschedule you to avoid running late for following patients. We ask that you let us know if something upsets you, so we have a chance to apologise and put it right. We ask that if you are happy with the care that you receive you recommend us to a friend, family member or work colleague and that you let us have a written testimonial for us to show to other patients about to undergo a similar dental experience. Payment for Treatment Payment can be made by cash or Visa or MasterCard or debit card or through a financial arrangement if agreed by the dentist. We ask that you settle your account on the day that you receive your treatment. Should an appointment need rescheduling then we need at least one full working day notice to allow us a chance to offer this appointment to another patient. Failure to provide notice may result in a cancellation fee being charged to offset the cost of the lost appointment. Should any monies be outstanding we will send two written reminders after which we will place the account in the hands of a debt collecting agency and the fees will be passed on to you. Interest at 2% per month or part thereof will be charged on outstanding accounts not settled after the first reminder. Guarantees The treatment you receive will be appropriate to your level of need and will be completed to the absolute best of our abilities. We will guarantee the treatment we provide for 12 months. Should it fail with in this time then we undertake to redo it free of charge, however we cannot be held responsible for any consequential loss. All guarantees are conditional on the patient attending to see the dentist and hygienist at the prescribed intervals in order to prevent further dental disease. The guarantee applies to the restoration provided. It does not cover any deterioration in the underlying tooth, e.g. if a root treatment is required or part of a tooth crumbles necessitating a more extensive restoration. The guarantee does not apply if the treatment plan has an acknowledged higher failure rate that the dentist made clear at the outset of treatment. The guarantee does not apply where the damage to the restoration is due to trauma, in which case it may be covered by any existing dental insurance. Dental treatment cannot be expected to last a lifetime without needing replacement through wear and tear and changes in your mouth. Excessive clenching or grinding, frequent sugar intakes, smoking and poor oral hygiene can all affect the longevity of any completed work. |
